✓ Register your account on Dental eServices using your ZimVie Customer Number and Billing Zip Code.
✓ Navigate to the 'Account Manager' tab in your eServices account and select 'My Profile.'
✓ Click on 'Enroll in Loyalty Program,' and add the 1-year membership program to your shopping cart. During checkout, include the contact information for our 'Loyalty Coordinator' and finalize your order.
✓ Complete the ‘Get Started’ Links, including the 12-month enrollment fee of $299
